This is Silurian Software's Site Vigil Scheduler

It allows you to view the status of monitoring operations and set up new configurations for one or more web pages and sites.

Site Vigil can be removed from your system by using Add/Remove Programs from the Control Panel and then selecting Silurian Site Vigil from the application list.

You can check for upgrades from within the Options License tab or going to the web site.
